Our friendly, knowledgeable staff is here to help you in every way possible. If you have any questions or concerns regarding office policies or procedures, please let us know.
Appointments
Office visits are by appointment only. Please notify us at least 24 hours
in advance if you are unable to keep an appointment. Our office staff
and other patients sincerely appreciate your cooperation.
Payment
Payment for all office consultations and minor procedures is due at the
time services are rendered. A nominal fee is charged for all initial visits
and consultations. This fee will be credited to your account should you
elect to undergo a cosmetic surgical procedure. Checks and major credit
cards are accepted. Financing assistance is available for individuals
who undergo major procedures.
Insurance
While it is the patient's responsibility to pay for all services rendered,
our staff will be more than happy to file insurance claims for anyone
who undergoes a surgical procedure. The Doctor belongs to several Preferred
Provider Organizations in the Chicago area. Please check with your PPO
to see if discounts apply.
